This form is only for children who have medical dietary requirements.

You DO NOT need to register with us if:

• your child does not have any dietary requirements.
• your child is vegan, vegetarian, avoids any meat or fish for religious reasons, or requires halal.

You only need to register with us if your child has an allergy, intolerance or medical condition that you need us to help manage as your school's catering provider.

We can provide allergen information or adapted menus to manage food allergies, intolerances or conditions such as coeilac disease or ARFID.

We can also provide nutrition information to help manage conditions such as diabetes or PKU.

We do not register children for food preferences or any personal or religious dietary requirements such as vegetarian, vegan or halal. Your school will help you to manage these so please contact them directly with this information.
